As far as the transformation of welfare systems is concerned, there are similarities as well as differences among Southern European countries – Greece, Italy, Portugal, Spain – with regard to both health and welfare services. Such differences are related to distinctive models of citizenship and socio-technical innovation. On the basis of a comparative and institutional approach, this paper compares the Italian case and the Greek one, with regard to home-based health care and welfare services. These two Euro-Mediterranean systems are compared on the basis of nine analytical dimensions, one of them being techno-assistance. The latter encompasses all the tools to be used in order to improve independence and self-reliance of patients at home. Such tools include telemedicine but are not limited to it. The analysis of indicators related to different dimensions suggests that specific trajectories of care transition are present.

Abstract
